Coronary Artery Bypass Graft (CABG) Surgery in India

Coronary artery by-pass grafting (CABG) or heart bypass surgery is a procedure in which healthy blood vessels are taken from other part of the body and connected above and below the blocked artery. This ensures improved blood flow to the heart and significantly lowers the risk of a heart attack by creating a new route for the blood to flow.

Minimally Invasive – Coronary Artery Bypass Surgery (MICS CABG)

This is a much newer and advanced technique of performing coronary bypass for the treatment of coronary heart disease. In this technique, a small 4 – 6 cm incision is made on the left side of the chest. This incision is used to approach the heart through the ribs without cutting any bones and the surgery is performed using laproscopy. The different techniques used are:

  • Minimally Invasive – Direct Coronary Artery Bypass (MIDCAB)
  • Off-Pump Coronary Artery Bypass (OPCAB)
  • Totally Endoscopic, Robot-Assisted Coronary Artery Bypass Grafting (TECAB)

Few hospitals in India follow a new technique and take artery grafts from chest and leg vein are taken only if many grants are needed. This saves the additional incision to the leg and faster recovery. Arterial grafts (mostly radial artery) are considered to be most durable grafts of this approach.

Advantages of Minimally Invasive Cardiac Surgery over the traditional CABG:

  • The biggest advantage of this technique is that, since no bones are cut, there is lesser pain and minimal impact on retaining function and breathing.
  • Shorter hospital stay i.e. usually the procedure lasts only 5 days unlike the conventional heart surgery that takes 9 days and hence the return to normal life is faster and easier.
  • Lesser blood loss eliminates the need for blood transfusion and hence no risk of any blood borne infections
  • Due to a small cut the risk of all infections is significantly reduced. Making this technique ideal for high risk patients such as diabetics and older patients with weaker resistance to infections.
  • Since the incision is just 2 – 3 inches it leaves a very small scar.

Limitations of Minimally Invasive Cardiac Surgery:

  • Since this is a new technique it requires specialised surgeons and at specialised hospitals.
  • The procedure is expensive compared to the traditional CABG
  • Strict patient selection criteria. Patients with extensive disease or very poor heart function are not suitable for this technique.

Endoscopic Vein Harvesting (EVH)

EVH adds further benefit to MICS CABG. State of the art equipment ensures quality with exceptional patient comfort. In this technique the leg vein is harvested to be used as a bypass conduit using an endoscope. Many studies have reported, that endoscopic vein-graft harvesting reduces post-operative pain, wound complications, duration of hospital stay, and a need for a large longitudinal incision.

Redo CABG’s

Redo coronary artery bypass grafting (CABG) occurs when patients who previously underwent CABG develop progression of their native coronary artery disease or graft occlusion that requires another surgical revascularization.

Factors Affecting the Overall Cost of the Treatment CABG:

  • The cost of CABG surgery is very high in Western countries.
  • India is known worldwide for its advanced medical facilities and promising technology for CABG surgery.
  • The average cost of surgery in India is less than that of Western countries.
  • The average cost of coronary Artery Bypass Surgery is around $ 4000 to $ 5500.

The various deciding factors that affect the total cost are:

Hospital Factors:

  • Type of Hospital( government or private)
  • Accreditation of the hospital ( JCI OR NABH)
  • Reputation and brand value of the hospital.

Surgeons Fee: The credentials and the experience of the cardiac surgeon also affect the overall cost of the surgery. Highly skilled and renowned surgeons increase overall expenses.

Geographic Location of the Hospital: The cost of heart bypass surgery also depends on the city or location where the surgery is performed. Major metropolitan areas like Mumbai, Delhi, and Bangalore translate into high medical expenses.

Technology Used: The technology used in the surgery affects the overall cost. The various technologies used in CABG are:

  • Beating heart CABG
  • Minimally Invasive CABG surgery
  • Off-Pump Coronary Artery Bypass
  • Endoscopic, Robot-Assisted Coronary Artery Bypass Grafting (TECAB)

Duration of the Hospital Stay: The length of the hospitalization affects the overall cost. Prolonged stay due to complications or other medical conditions may increase the overall cost.

Also, the type of room selected for the stay determines the cost.

Pre-operative Costs: Before the surgery, extensive evaluations are conducted to assess the patient’s condition. These evaluations may involve tests, consultations, and imaging studies. The cost of the evaluations is based on their complexity and the number of tests.

Post-operative Care: After coronary artery bypass surgery, patients require post-operative care like medications, monitoring, and follow-up care. The duration and the intensity of post-operative care can impact the cost.
